After the Columbia space shuttle disaster, a commission was formed to investigate what happened. The commission reported that a piece of foam that had broken off the shuttle during liftoff caused a hole in the wing, which resulted in the disaster. The commission also noted that the culture of NASA was also at fault saying that the “organization does not provide effective checks and balances, does not have an independent safety board, and has not demonstrated the characteristics of a learning organization.” This is just one (1) example of a major failing resulting from a complex system, NASA, interacting in multiple ways.
